Book excerpt: Assessment and evaluation processes for patient understanding of discharge instructions are essential for patient execution of self-care activities. During the acute phase of illness, patients are not able to learn or retain instructions well. The discharge process from an inpatient hospitalization does not allow for full evaluation of a patient’s understanding of their discharge instructions, though written instructions are provided. Implementation of a home visitation program by faith community nurses can contribute to improve the quality of the patient understanding of their discharge instructions. Utilizing the Teach Back Method to assess, evaluate and re-educate promotes patient understanding and execution of their self-care activities. The patient relationship, and experience, with faith community nurses demonstrates caring, as guided by the Watson Theory of Human Caring. This relationship yields a trusting environment that demonstrates patient advocacy through patient education and self-management. (American Nurses Association, 2012). The role of the faith community nurse in reviewing patient discharge instructions, provides a resource for the improved quality of patient healthcare by implementing the nursing process in patient education and understanding.
